"They came in around 8 p.m.," an insider told Life & Style for its new issue.
The five time Grammy Award-winning country pop singer-songwriter, who is a vegetarian, is reported to have started with ravioli and a beet salad before ordering an omelet for her entrée.
"They ordered a bottle of Malbec and two bottles of Evian water," says the insider. "There was definitely some hand-holding and petting. They even toasted with their wine."
Carrie, 25, and Travis, 36, also joined in a birthday celebration at a nearby table where "Carrie sang 'Happy Birthday!'"
She dated Dallas Cowboys quarterback Romo from Thanksgiving 2006 to May 2007. She then dated Gossip Girl hunk Chace Crawford from August 2007 to early 2008. The couple broke up over text.
Carrie Underwood has won a nomination at the CMA for Best Album of the Year for her album Carnival Ride.
Others competing in the category are Chesney's Just Who I Am: Poets and Pirates, Alan Jackson's Good Time, Strait's Troubadour and Brooks & Dunn's Cowboy Town.
The CMA Awards will air on November 12 8/7c.
